Under the direction of the Executive Director of Accountable Care, the Case Manager for the ACO Program will follow a collaborative process to assess, plan, implement, coordinate, monitor and evaluate the options and services required to meet an individual's health needs. The Case Manager for the ACO program is responsible for the identification, evaluation and coordinator of all applicable health care services provided to the attributed ACO population and patients identified as high risk with two or more chronic health conditions.

Minimum Requirements / Work Experience * Three years in acute care setting * Previous ACO Program preferred * Understanding of privacy and HIPAA standards * Proficient computer use (includes typing, Internet, MS Office, Analytics Software) * Able to work independently
Education / Licensure / Certification * Current California Nursing license in good standing (RN preferred) * 3 years of case management experience and or CCM (preferred)
