HCT seeks an experienced professional to serve as the Case Management Leader, at a 200+ bed hospital in California. The Leader’s responsibilities include, but are not limited to:
•providing daily, operational oversight including managing to performance metrics,
•mentoring the Case Management team,
•leading on-going process improvement efforts,
•collecting, analyzing, evaluating and presenting clinical findings and operational data, and
•supporting policies and standards.
REPORTS TO: Vice President, Care Coordination
•Education: Master’s Degree in nursing (MSN) required; Masters in Social Work or Social Welfare preferred
•Experience: 5 to 7 years of Case Management or Utilization Management experience in a healthcare organization; 2 to 4 years in a supervisory role
•License: California RN license required or licensed in Clinical Social Work in California
•Certifications: BLS or other advanced life support certifications; CPI- Crisis Prevention Intervention Training preferred
•Computer Skills: Multiple EHR systems experience, working knowledge of Case Management information systems, and Microsoft Office suite preferred
