Clinical Practice: In accordance with the National Scope and Standards of Diabetes Education, as well as the National Standards for Diabetes Education. The Diabetes Nurse Educator assesses the needs of patients with diabetes and designs, implements and evaluates an individualized teaching plan for these patients. Knowledgeable in all aspects of diabetes care, and acts as resource for patients, families, community, and other stakeholders and service providers. Integrates knowledge, skills, and experience to meet the multi-faceted needs of diabetes patients and families throughout the continuum of care.
Program Development and Promotion: Plans, develops and implements new programs to meet the needs of the patient population and enhance diabetes self-management education within Inova. Participates in activities that promote and expand the reputation of Inova's comprehensive network of services.
Teamwork: Interacts with interdisciplinary team of health professionals. Communicates effectively and works cooperatively with other team members. Utilizes the unique contributions of each discipline and applies an integrated team approach to improve patient outcomes.
Quality/Best Practice: Utilizes standards, and evidenced based practice for delivery of care and education. Incorporates new data and learning to continuously enhance patient outcomes. Applies quality improvement methodology to improve overall diabetes services.
Professional Development: Ensures improvements in diabetes practice by assuming responsibility for self-development in life-long learning. Provides direction and guidance to others regarding practice, serves as a resource, preceptor, and mentor. Leadership skills demonstrated in decision-making and problem solving. Bilingual Spanish strongly preferred. Classes taught in Spanish and English.
***Bilingual Spanish strongly preferred. Groups taught in Spanish and English. For other languages, translation services are available to assist 1:1 patient teaching. Experience: 1 to 2 years RN work experience or 250 hours of DSME/T.
Education: Bachelor of Science in Nursing from an accredited School of Nursing
Training: 15 CE units per year in DSME/T
Certification: BLS. Must complete CDE within one year of employment
Licensure: Current RN Licensure.
Travel: CHCN has 2 satelite locations (1 clinic in Alexandria and 1 clinic in Reston) CDE will be required to travel to each of these locations 1 day per week to provide patient education/ encounters.
Special Skills: Bilingual Spanish strongly preferred. Effective group management, classroom teaching, and communication and counseling skills. Good clinical judgment and problem solving skills. Proficient in email, PowerPoint, Excel and word processing software.
