Our Continuity of Care Supervisor is responsible for the supervision and operations of the Social Services Department. The Social Services Department acts as the liaison for involvement of the family in recovery planning and serves the primary function of discharge planning by addressing facility discharge barriers. Provides oversight for the discharge process involving step down to civil hospital placement and the community via conditional release plans. Coordination of communication with the Managing Entities, forensic specialists, and community placement liaisons is a primary duty.
Master’s degree in Social Work
Experience with discharge planning and social security benefits applications.
Experience in forensic/correctional/inpatient settings is highly preferred.
Two (2) years of experience dealing with severe and persistent mentally ill patients, is necessary.
Three (3) years of experience working with the court systems and personnel system, preferred.
Discharge planning experience.
