Job Duties & Responsibilities
Collaborates with patients, physicians, office staff, ancillary departments within the company, and community agencies to maintain accurate and timely data exchanges between all entities involved in the patients’ care.
Provide care coordination services to identified clients; making referrals to appropriate community resources to assist clients in dealing with the barriers they face, related to addiction; such as counseling, financial planning, housing issues, legal assistance, transportation, etc.
Meets with and assesses each new CSC patient for needs and barriers
Group co-facilitation with providers.
Obtain appropriate releases with each referral that is completed
Coordinates patient appointments, referrals, and transportation needs for treatment within the company and community.
Facilitate placement for needed alternative or higher level of care as agreed upon with the Interdisciplinary Team; maintaining contact with these placements to provide continuity of care upon discharge.
Maintains effective, pro-active, and superior follow-up communication with patients, providers, office staff, and ancillary departments within the company and community.
Maintain an accurate and updated record of individual client compliance with community counseling providers.
Provides administrative support for clinical site team members, including but not limited to scanning, mailings, filing, copying, and faxing applicable documents and records.
Enters all required data into the EMR system daily.
Maintains excellent level of documentation and note keeping.
Maintains compliance with all levels and forms of patient confidentiality.
Attends regular meetings or conference calls, including but not limited to involving Interdisciplinary Teams and collaborating community agencies; completing all required documentation in the EMR.
Provide crisis intervention, as necessary.
Maintain individual schedule of assigned patients
Provides cross coverage for Practice Administrators, as needed.
Bachelor's Degree in a Human Services field required.
Excellent written and verbal communication, organizational and customer service skills.
Knowledge of community resources, networking, and referral processes.
Knowledge of and proven experience navigating and documenting in an EMR system.
Ability to work independently with minimal supervision.
Ability to handle multiple tasks simultaneously and prioritize based on severity of need.
Experience in a Case Management, or Care Coordination setting.
