A registered nurse who provides ongoing assessment, planning, intervention, treatment and evaluation for REACH patients seeking care for psychiatric/mental health/behavioral problems and/or substance abuse. Job responsibilities include the following. Other duties may be assigned.
The REACH Program serves YOUTH through ADULT -- IOP & Medication Management Services offered.
1.Demonstrates advanced clinical skill in interviewing, observing and collecting healthcare information during time of assessment.
2.Prescribe and monitor psychotropic medications while providing diagnostic impressions and specific treatment recommendations.
3.Provide therapeutic groups outpatient population.
4.Performs psychiatric evaluation and provides clinical diagnosis.
5.Clinically assess and evaluate REACH patients who present with emotional, behavioral and or psychiatric health needs.
6.Collaborates with Multi-disciplinary support team to develop treatment plans for patients.
7.Completes required documentation in a timely manner and adheres to the departmental standards.
8.Assures patient's rights and safety are maintained at all times.
Advanced practice registered nurse with a state-approved master’s degree in nursing (MSN) at minimum.
Experience in psychopharmacology required. Minimum 3 to 5 years of clinical experience related to field of Psychiatry required. Lifespan certification a plus.
Advanced practice registered nurse who has successfully fulfilled the state’s requirements in terms of clinical hours, and the completion of an advanced practice certification exam. Must hold active APRN licensure and maintain compliance with state requirements.
