Job Summary The Activity Care Associate - ADHC is responsible for assisting with the daily activities in the Adult Day Health Care Program. Greets the frail, elderly, and physically handicapped clients and family members each day, provides and assists with meals and snacks, assists with activities, transportation and maintains an orderly work area. Assists participants with personal care services such as toileting, bathing, feeding, showering, changing and transferring of participants. Assists RN with the taking and recording of vital signs and other personal care services as needed.
1.Assists with, or provides personal care services as instructed by the ADHC registered nurse. Offers and provides meals and snacks to clients as directed including feeding assistance and monitoring of intake and special dietary requirements.
Ensures that all participants' basic needs are being met on a consistent basis as determined by the Client Service Plan observation and documentation. Takes all steps necessary to ensure safety of participants and reports any significant medical information to nurse as evidenced by documentation.
Follows proper procedures and health sanitation standards at all times in the area of food preparation, serving, feeding and storage of food items as evidenced by documentation and observation.
Follows policy and procedure for accurate count of actual meals and snacks served for CACFP billing.
Complete health monitoring according to licensure and contracts standards as evidenced by documentation.
Participates in the data collection for Quality Assurance
Participates in monthly staff in-service to remain current on the geriatric population needs
2.Assists with program activities for participants including the leading and monitoring of participants in a wide range of areas.
Works with activity coordinator to ensure that all scheduled activities effectively meet the needs of all participants at a variety of functional levels as evidenced by feedback and observation.
Works with other activity staff to facilitate the intergenerational programs.
Assists the activity coordinator with client chart documentation.
Leads activities, outings, and events under the leadership of the activities coordinator.
1.Coordinates and ensures a wide variety of daily activities/services are delivered according to established schedules and procedures, including showers, transportation, and ambulating and personal care assistance.
Works closely with other program staff to ensure effective coordination of scheduled services as evidenced by feedback and observation.
Assists the program coordinator in maintaining a safe, quality program through complete and accurate Performs other duties as assigned.
