Palmdale Regional Medical Center serves the region by providing high quality medical services to residents of the Antelope Valley, Santa Clarita Valley and surrounding communities. With the recent addition of the Rehabilitation Institute, the hospital features 184 licensed acute care beds as well as inpatient and outpatient surgery, cardiac services featuring a STEMI Receiving Center (heart attack) and a 35-bed 24-hour emergency department. The Center for Wound Care provides treatment for people with chronic non-healing wounds. This comprehensive wound-healing service includes hyperbaric oxygen therapy.

The clinical care coordinator facilitates the care and services of patients with identified social services or case management needs across the continuum of care. The clinical care coordinator works in collaboration with the healthcare team to ensure timely, effective and efficient delivery of appropriate healthcare services to Acute Rehabilitation Center patients. The clinical care coordinator is a liaison among the patient, the family, and the healthcare team and community agencies. He or she works within a team to develop, facilitate and implement a comprehensive plan of care for the patients based on ongoing assessments of the patients’ needs and condition. The clinical care coordinator specializes in the coordination of care and discharge planning for patients with complex or specific social services needs.
ESSENTIAL KNOWLEDGE / SKILLS
Must successfully pass any pre-employment assessment(s) required by the facility.
MINIMUM ESSENTIAL EXPERIENCE
3-5 years' experience in case management or medical social work required.
MINIMUM ESSENTIAL EDUCATION
Registered Nurse or Master's degree in Social Work from CSWE accredited institution required.
REQUIRED LICENSURE / CERTIFICATIONS:
Current RN California license or MSW required (ACSW or LCSW preferred)
