Position Summary The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post-acute services for patients and assist with tasks that do not require a clinical license or degree.
Primary Duties Bachelorâ��s degree preferred. Paramedic, EMT or Nursing Assistant license preferred. Acute hospital experience preferred. Skills require excellent organizational skills, excellent verbal and written communication skills, demonstrate problem solving skills and computer literacy.
Physical demands: â�¢ Frequent sitting. â�¢ Frequent use of fine motor skill (typing/data entry). And reach with arms and hands. â�¢ Frequent standing, walking. â�¢ Occasional stoop, kneel or crawl. â�¢ Lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
Working Conditions: â�¢ May be exposed to infectious diseases. â�¢ Interacts with patients and visitors.
Minimum Education/Certification/Experience: â�¢ High School diploma or equivalent required. â�¢ Bachelorâ��s degree preferred. â�¢ Paramedic, EMT or Nursing Assistant license preferred. â�¢ Minimum of 1 year Acute care experience. â�¢ Skills required include excellent organizational skills, excellent verbal and written communication skills, demonstrated problem solving skills and computer literacy.
