Work Hours/Shift:

Full Time Days

Monday â“ Friday

8:00AM â“ 5:00PM

You Will Be Responsible For:

• Timely, accurate, and thorough completion of medical documentation upon patient intake (including identified standards for documentation â“ i.e. mammogram, colonoscopy, smoking, etc.
• Ability to give immunizations
• Ability to utilize computer/computer software for pertinent patient documentation, providing documentation to patient as needed (i.e. patient plan).  This includes pertinent training as necessary from time to time
• Adhere to policies/procedures for patient medications and testing â“ refilling patient medications according to protocols (SMPG policies) only as appropriate
• Electronic communications with patients, when desired (in keeping with established policy standards)
• Participate in group huddles to assure continuity of care
• Excellent communication (both written and verbal)
• Timely management of patient needs/requests (including phone calls) in accordance both with policy and established PCMH standards
• Maintain pertinent logs of work performed as detailed by Clinic Manager/Director
• Adhere to clinic specific competencies (checklist)
