The VA Maryland Health Care System (VAMHCS) is a Level I tertiary multi campus teaching facility affiliated with the University of Maryland, School of Medicine and over 100 educational institutions. The VAMHCS consists of the acute care Baltimore facility; the multi building Perry Point campus ; the Loch Raven campus ; and six urban and rural outpatient clinics. The Patient Aligned Care team ( PACT) LPN serves as the essential role of being the first direct care clinician to make contact with the veteran at their appointment. The LPN checks vital signs, patient weight, interviews the patient for clinical reminder resolution, medication administration and regular communication with the PACT RN and/or provider. Duties also include ancillary testing for glucose, hemoglobin A1C and PT/INR as well as 12-lead EKG as needed. The LPN interacts with patients regarding health promotion and disease prevention regarding immunizations and follow up for chronic disease state management. There is also responsibility to facilitate clinic flow and efficiency. Having the LPN on the team will relieve the RN of tasks that compete with the priorities of care coordination and enhance patient access by phone and face to face nurse visits and accommodating patients who "walk-in" for care of questions.
