The Patient Safety Manager (PSM) demonstrates leadership and creative approaches to the management of complex patient care with a focus on Patient Safety and High Reliability Organization (HRO) processes. The Patient Safety Manager collaborates across service lines and departments and with diverse disciplines in order to ensure the delivery of safe patient care and works to assure continuous survey readiness, compliance with national office program requirements and compliance with National Patient Safety Goals. Duties include but are not limited to: S/he communicates with appropriate staff regarding the identification of safety and risk issues. The incumbent completes daily reviews of electronic incident reports and follows up as appropriate. Responsible for analyzing data from monitoring activities and presenting it to the appropriate audience in useable appropriate formats. S/he manages databases (JPSR, SPOT, PSAT, Alerts and Recalls, etc.) including entering data and pulling reports to assist the facility in decreasing adverse events. The incumbent participates in activities related to patient safety such as leading and advising Root Cause Analysis (RCA) teams, the completion of Proactive Risk Assessments (PRAs), tracking of alerts and recalls, and action plans related to either potential or actual adverse events. S/he also utilizes constructive problem solving and management of conflict related to patient safety throughout the facility.
