Provide pharmaceutical care services to the pa tients of the DukeUniversity Health System including, but not limited to oversight,monitoring, preparation, dispensing and proper documentation ofpatientmedication therapy as well as providing drug information, patie ntinformation and oversight of information and dispensing systems; ensur ecompliance with Federal, State and local laws and regulations as well a sproper storage and documentation. Mayalso advise physicians on matterspertaining to drug usage and control; participate in the development andd elivery of continuing education programs for Pharmacy, Patient CareServi ces, Medical Staff and students.
Revie wand monitor patient's medication therapy and orders written byappropria te caregivers for safety and efficacy considering specificpatient condit ions and health status; advise patient care team of issueswith, or possi ble enhancements to, patient's medication therapy anddocument such inter ventions. Prepare, label and dispense medications forpatients in accorda nce with physicians' prescriptions; ensure compliancewith Federal, State and local laws and regulations pertaining to thedispensing of drugs andn arcotics and the maintenance of requiredrecords.Maintain appropriate rec ords of dispensing and oversee and maintain thesystems that charge orcre dit for medications and medication treatments.Provide drug information a nd educate caregivers, patients and studentsregarding medications andthe ir proper use and monitoring. Oversee andmonitor all aspects of themedic ation process; collect data and presentreports and recommendationsregard ing issues with the medicationsystems.Ensure that appropriate medication s are available for patients andproperly procured, prepared, stored and maintained.Supervise the process and technical personnel who assist in p reparation,maintenance and dispensing of drugs and supplies.Provide supp ort, education and oversight for investigational medicationsfor patients ; provide information regarding their preparation, labelingand availabil ity within the medication system; maintain appropriate andrequiredrecord s and documentation for investigationaldrugs.Providespecializedpharmaceu tical services to improve drug usage andtherapeutic outcomes including a dvising physicians on issues concerningdrug therapy, the inherent toxici ty of drugs and side effects, as wellas assisting in the prescription of appropriate doses.Contributes to the education of students and pharmacy residents.Maintain liaison relationships with medical and nursing staff ; providetimely information pertaining to pharmaceutical supplies, drug usage andcompatibility, state and federal regulations regarding drug con trols,and Joint Commission standards.Conduct and evaluate medication his tories, assess compliance and suggestmodifications so as to achieve desi red outcomes; instruct patients inthe proper use of prescribeddrugs; par ticipate in patient care roundswith multidisciplinary teams to evaluatepatient progress.Individualizemedication regimensusing sound principles , accounting forpharmacodynamic and pharmacokinetic variations in drug a bsorption,distribution, metabolism and elimination with responsibility f orestablishing and continually improving the delivery of pharmaceuticalc are to patients within areas of direct responsibility and assistingother s in the department with the same.Conduct and participate inresearch inc luding, but not limited to,performing as the primary investigator or co-investigator for researchprograms which will impact on thedelivery of qu ality care or examinethe pharmacoeconomic impact of providing care to th e patients in theDuke Health System.Perform other relatedduties incident al to the work described herein.

Level Charac teristics
Doctor of Pharmacy Degree OR BS Pharmacy Degree
With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required.
Hospital pharmacy experience with a working knowledge of the EPIC EHR strongly preferred.
