Geriatrics and Extended Care Medical Practitioner will provide high quality patient centered care to veterans mainly in inpatient long-term care unit (Community Living Center), Geriatrics consults and screening referrals, serve as a teaching attending for house-staff from Baylor College of Medicine and MD Anderson Cancer Center, rotating through MEDVAMC long term care programs. Clinical Duties: The exact privileges and procedures for a particular provider will be approved by the Professional Standards Board. Shares responsibilities for geriatric consult management for CLC (Long Term Care/ Skilled Nursing Home) Geriatric consult, house staff teaching etc. Serves as a geriatrician in Geriatrics and Extended Care service, more specifically as a direct patient care attending in an inpatients long-term unit. Work collaboratively with APRN and/or PAs and specialists from other disciplines. Other Duties as assigned by supervisors based on organizational needs and employee competencies. Administrative Duties: Leadership responsibilities: participate in staff meetings, hospital committees Long-term Care Institute (LTCI) and Quality and Process improvement), and peer review Demonstrate effective communication skills and be an effective change agent Oversight of Non-VA Nursing home care including but not limited to quality of care. Maintaining VA training requirements, timely completion of documentation and upkeep of credentials required for privileging and re-privileging. Participate in the FPPE/OPPE process. Adheres to employee health requirements. Participate in Clinical Pertinence Reviews (routine medical record reviews). Adherence to tour of duties, timely requests for leave in VISTA and for clinic cancellations. Clinical Management including indirect patient care: open encounter & open consult management, CPRS documentation rules and requirements, adherence to clinic procedures for patient scheduling & access, ensuring staff productivity, oversight of relevant performance measures, and participation in the development of the new focus on the Cultural Transformation, Utilization, Quality, and Patient Safety oversight, process improvement (system redesign). Arranging physician coverage and communicating the plan of coverage to the team members. Serving as back up coverage for other GEC providers. Education: Practitioners are expected to show effectiveness in teaching, monitor and coordinate educational activities, and comply with necessary trainee supervision. This position is subject to 2 year of probationary period per guidance per regulatory requirement Work Schedule: M-F; 8:00am-4:30pm; other times as required subject to change based on agency needs.
