This position is located in the Pulmonary Section, Medical Care Line at the Michael E. Debakey VA Medical Center. Primarily works in collaboration with Telehealth Rehab providers and staff from Medical Care Line to ensure effective, efficient, and patient centered Physical Therapy services are provided for patients referred. Responsible for screening and enrolling patients into the program, and their delivery of care throughout the twelve-week course. Designs plan of care and delivers care through Telehealth Service to address common health consequences in post-COVID patients (e.g. fatigue, shortness of breath, deconditioning, falls and impaired mobility). Participates in the complex decision-making process of developing the Telehealth Rehabilitation Programs for patients with post COVID symptoms at many levels including independently reviewing medical records, assessing patients, providing physical therapy to patients and retrieving clinical outcome data. Performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors. Independently interprets consults, findings from examination and evaluation, selects appropriate test and measures, and integrates findings into the physical therapy plan of care for the full range of patient populations. Appropriately establishes a diagnosis within the scope of physical therapy and identifies the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated. Determines accurate physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence. Communicates effectively with patients, families, caregivers, healthcare professionals to facilitate the interdisciplinary treatment process. Reviews the medical record and independently evaluates each patient to determine functional deficits and the best method to accomplish rehabilitation goals. Evaluations include medical history, results and interpretation of appropriate functional tests and measurements such as: range of motion, muscle strength, sensory deficits, coordination, functional mobility, pain scales, etc. Utilizes standard and non-standard methods in the evaluation and interpretation of a wide variety of physical functional levels. Develops an individualized patient centered treatment program with each patient including functional, measurable, and timed short and long term goals as appropriate. Implements, in a timely manner, progressive treatment programs to meet goals, taking into consideration the specific rehabilitation needs of each patient. Applies recognized Physical therapy treatment procedures correctly, safely, and modifies these techniques in such a manner as not to interfere with any traction devices, surgical drains, catheters, immobilizing appliances, respiratory aides, or monitoring devices. Independently adapts and modifies treatment programs, to better meet short term and long term goals. Recognizes adverse reaction to treatment, significantly altered physical or mental status of patients, and reports to appropriate medical authority. Documents initial findings, treatment goals, equipment recommendations, treatment provided, patient progress, patient/family education, and discharge summary in patient's medical record within established guidelines. Evaluates for and recommends appropriate assistive devices and special equipment as indicated. Accurately completes timely Prosthetics consults for recommended items. Fits devices appropriately for patient needs and instructs patient/care provider in the use of these devices. Develops appropriate home program with patient, as needed, prior to discharge. As indicated, instructs family or care provider in ways to assist patient to carry out the program. Completes patient and/or family training as indicated. Participates in team conferences, rounds, discharge planning meetings, specialty clinics, other assigned medical meetings. Observes infection control procedures, safety and treatment precautions pertinent to the patient's condition or disease process, and the specific treatment technique or procedure being utilized. Shares new and evidence based techniques and information with all appropriate staff. Provides assistance and guidance for less experienced staff in physical therapy treatment. Assists with educating/training of other students and less experienced staff.
