Serve as a Home Based Primary Care (HBPC) Staff Physician located within the Miami VA Healthcare System, assigned to the Geriatrics and Extended Care Service. The HBPC Staff Physician practices in the homecare setting, and visits are made to patient residences at home. Veterans may be examined in more acute settings occasionally to assess appropriateness for HBPC care. The HBPC Staff Physician works collaboratively with members of the HBPC service to provide high quality cost effective care to a population of veterans who are experiencing geriatric syndromes or are at risk of hospitalization due to chronic progressive diseases. Duties and responsibilities include, but are not limited to: Visit all assigned new patients admitted to the program in their homes, and directly provide or supervise medical care received by HBPC patients in accordance with the health care goals of the veteran and his family or legally authorized representative. Serves as supervising/collaborative physician to Advanced Practice Registered Nurse (APRN) who serve as primary care providers. Provide education and mentoring to other interdisciplinary team members, patients and families, foster caregivers, students including residents, fellows, and interdisciplinary learners. Participate in activities that advance the practice of home care through local and national involvement within and outside the VA Health Care System. Work collaboratively with VA staff and community partners and participate in performance improvement activities and research where appropriate. Keep the HBPC team apprised of medical care advances and practice standards. Provide cross coverage for HBPC Physicians during absences as well as night call for coverage of patients assigned to inpatient services of the CLC and Palliative Care unit of the GEC which is shared among all GEC physicians. This may include occasional weekend and/or holiday coverage. Perform administrative functions, clinical pertinence chart reviews, biannual performance reviews, and signatures of home health and hospice orders as well as death certificates. Provide coordination of medical students and residents rotating through HBPC. Work Schedule: Monday-Friday, 8:00AM - 4:30PM. Relocation/Recruitment Incentives: Not Authorized.
