Provides an integrated continuum of care for patients from preadmission to post-discharge. Completes required evaluations. Completes Individualized Treatment Plans. Provides group, individual and family therapies. Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. Communicating with patients and their families around plan of care. Communicating and collaborating with physician and other team members.

Minimum Education: Masters degree in Social Work required.
Minimum Experience: 1 to 2 years of job-related experience preferred, including experience as a licensed clinical social worker, experience in an acute care setting, knowledge of federal and state laws and CMS requirements and calculations.
Required Certifications/Registrations/Licenses: S.C. licensure in Social Work. BLS upon hire, CPI within orientation period.
