The Patient Safety Program Manager Facility Program Manager is a licensed pharmacist who collaborates with all services including pharmacy facility leadership to support medication safety at VA Salt Lake City Health Care System. Some of the duties required are, but are not limited to: Performs duties directly contributing to the administrative management of the facility health care delivery system, and is responsible for planning, organizing, developing, managing, and evaluating the medical center's patient safety program. Performs Root Cause Analysis activities such as: SAC classification Provides RCA "just in time" training. Identifies RCA Team members. Supports RCA Teams, e.g., RCA Advisor. Develops competencies in team functions. Ensures that RCAs are complete, that all technical requirements are met, and all implementation requirements are accurately identified. Familiar with and proficient in the use of SPOT, VHA's national patient safety software application. Supports aggregate review process, functions in the role of advisor. Follows-up on RCA Actions/Outcome Measures. Resident expert on confidentiality rules as they apply to the patient safety process Performs general patient safety programmatic functions including: Serves as the facility point of contact for Patient Safety Alerts/Advisories, including tracking system actions. Serves as member of various medical center committees, as appropriate to the position, e.g., Safety/Environment of Care, Pharmacy and Therapeutics, etc. Conducts periodic environmental patient safety inspections. Collaborates with Biomedical Engineer, Occupational Safety Manager, Industrial Hygienist/ Infection Control, Pharmacy Management, etc. Shows leadership in publicizing and promoting patient safety successes and activities, e.g., town meetings, poster sessions, news flashes, awards, etc. Disseminates Patient Safety Reporting System (PSRS) program information. Provides new employee patient safety orientation and training. Develops facility patient safety training program. Acts as the Point of Contact for facility patient safety issues with NCPS. Serves as the facility expert on the VHA Patient Safety Improvement Handbook. Initiates and advises on Healthcare Failure Mode Effects Analysis (HFMEA) evaluations. Familiar with and addresses results of periodic safety culture survey with action plan for facility. Acts as liaison on patient safety issues to facility Quality Manager, Joint Commission, and others. Evaluates the RCA process as it functions in the facility and responds to improve it as appropriate. Develops background and management options available to deal with complex medical center planning and implementation problems related to the facility's patient safety program. Plays a critical support role in developing and coordinating internal review systems to assure that both clinical and administrative patient safety activities are in compliance with agency and accrediting standards and regulatory requirements.
