The Staff Physical Therapist is assigned to the Physical Therapy Department within Pain Management and Rehabilitation Service (PM&RS) at the VA Northern Indiana Health Care System (VANIHCS). The Staff Therapist is responsible for completing neuromusculoskeletal evaluations and treatments. This position will be at the Community Based Outpatient Clinic (CBOC) in Peru, IN. Major Duties and Responsibilities include but not limited to: Assists in planning and developing various treatment programs to meet patient needs, as requested by PM&RS supervisor. Assist with orientation of new Physical Therapy staff. Establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis at the VANIHCS. Assists in the development, planning and implementation and monitors of quality of care with respect to PM&RS as part of the comprehensive health care services delivered to the patients within the NIHCS. Conducts record keeping related to patient care documentation and quality improvement (QI) data collection as specified by their Immediate PT Supervisor. Responsible for the examination and treatment of patients referred to the Physical Therapy Department. Demonstrates the teaching skills required to provide education to patients, family members, caregivers, employees, students, and coworkers. Functions autonomously in carrying out patient education, specialized treatments, and applying physical therapy procedures for which competency has been established and in accordance with laws or regulations and APTA documents. Modifying the treatment plan, therapeutic activities and interventions based on the changing needs, goals and performance of the patient. Performs competent written documentation of initial examinations, progress notes, and discharge summaries. Entering consults within the PM&RS Service (i.e., to Occupational or Recreational Therapy) or to the Prosthetics service for rehabilitation equipment in accordance with PM&RS and medical center policies. Reviews and co-signs notes written by the PTA to whom the PT has delegated patient care or for patients seen by the PTA within the PT-DME clinic. Acts as Clinical Instructor for affiliating schools for physical therapy. Educates other facility personnel in the scope of PM&RS services and programs involving a variety of health care specialties. Maintaining state licensure and completing all VA Medical Center mandatory training in a timely manner. Completing Encounter information in accordance with VANIHCS guidelines. Assures the cleanliness, maintenance, repairs, and successful operation of all Physical Therapy equipment, treatment areas, and operational supplies.
