The staff RN Care Manager interprets the philosophy and goals of Nursing Service to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop sound relationships with clients, their caregivers and other professional and supportive personnel who provide client care. There are currently two vacancies to be filled. The duties of the RN Care Manager include, but are not limited to: Demonstrates ability to function effectively in charge nurse, tea leader, or other leadership roles. Applies problem-solving skills at the unit or team level, using the team approach to identify, analyze, and resolve problems. Uses sound clinical judgement in assessing, planning, and implementing. Documenting and evaluating nursing care. Provides a full range of nursing care to clients with complex care needs, assuming responsibility and accountability for nursing actions. Provides assessment of care for scheduled and walk-in Veterans, including follow-up when necessary. Performs assessment and prioritization for patients who present to the clinic requesting same day appointments. Performs telephone triage. Acts as a client advocate when client self-determination is in question. Maintains an ethical nursing practice by assuming a leadership role in addressing ethical issues related to patient care. Uses discretion in confidential matters and leads others in appropriately safeguarding confidential information. Applies knowledge of diverse nursing procedures and takes leadership role in using safe, cost-effective measures to provide care. Participates in evolution of RN Care Manager role through development of processes, templates, protocols, and in collaboration with appropriate leadership. Initiates and resolves work flow problems taking into account safety and patient care needs. Identifies the impact of changes in level of care on resources. Participates in professional and staff development activities. Evaluates own practice utilizing professional standards of practice and relevant regulations. Attends all PACT training events and demonstrates knowledge and implementation of acquired information. Self-directed in evaluating progress toward professional career goals. Demonstrates ability to work effectively with patients, significant others, professional and supportive personnel who provide patient/supportive care, initiating/requesting additional support from any team provider for optimal quality care. Participates in evaluating and communicating standards of nursing practice. Participates in the formulation, review and revision of nursing policies and procedures. Develops, conducts and participates in patient groups. Assumes leadership role in identifying policies, procedures, standards, guidelines and protocols requiring review or revision. Collaborates with other disciplines and caregivers and uses group process for decision-making and interdisciplinary problem solving. Communicates in a positive, courteous and friendly manner with patients, families and visitors. Shares clinical expertise with professional and support personnel. Demonstrates leadership by sharing knowledge with colleagues and/or students to enhance their professional growth, development and practice. Identifies problem areas for monitoring and evaluation and is active in analyzing findings and changing practice based on quality improvement findings. Demonstrates leadership at the unity level in quality improvement activities. Work Schedule: Full Time; Monday-Friday, 8:00am - 4:30pm with potential for off-tour and weekends.
